Discipleship Now (DNow) is a conference for students from 6th-12th grade hosted at Calvary Baptist Church. This year's theme will be salvation. The event will include 3 sessions throughout the weekend, each one addressing the topic of salvation. The weekend will also be full of fun and fellowship including a trip to Jaeger's Laser Tag in North Kansas City on Friday night.

What: A conference for students grades 6-12.

I do herewith authorize emergency treatment be given if necessary only after reasonable effort has been made to reach us/me the parent(s) or guardian.

I undersigned do hereby release and agree to hold harmless the Calvary Youth Group leadership and Calvary Baptist Church from any and all liabilities or claims for personal injury or illness which may be incurred by my child while attending and participating in the event.

I further release Calvary Baptist Church and its staff and leaership from any claims that I may have against them as a result of my child's picture being used in promoting Calvary Youth Group within Calvary Baptist Church.
